At Home Hospice

For those who are able to and/or prefer to receive hospice care in the comfort of their own home, they can expect to receive a high degree of professionalism and patient care:

Registered Nurses

Registered Nurses will make visits to provide symptom control, pain management, and provide education on disease progression. The Nurses will assist with an array of functions from taking vital signs and providing medication management; to in-depth education on illnesses and how to better control symptoms. Nursing services are available 24 hours per day and serve as a liaison between their doctor and the patient.

Social Workers

Social Workers assist with counseling, financial resources, and future planning. These educated professionals are available 24 hours per day to provide assistance with the psychosocial, financial, and emotional needs of patients and families. They provide links to community resources and grief counseling services.

Certified Nursing Assistants

Certified Nursing Assistants are available to assist with activities of daily living. These include bathing, eating, shaving, and mouth & nail care, as well as assisting caregivers with light housekeeping.


Chaplains are available to assist patients with spiritual counseling no matter the denomination or affiliation. Chaplains are available 24 hours per day and are specially trained to address concerns and provide bereavement counseling.
We even have loving volunteers who run errands, read books, build ramps and spend time with our patients.  All home care is overseen by the patient’s own attending physician and the hospice medical director


Community Hospice accepts any patient who is terminally ill and meets hospice qualification guidelines regardless of their ability to pay. Community Hospice does not bill the patient or their family for hospice services.

By choosing home hospice a patient can receive the services needed to keep them in their home environment.

  • Hospice provides and pays for medications needed for comfort and those required for the medical condition.
  • Hospice provides all medical equipment and supplies such as hospital beds, oxygen, wheelchairs, diapers, gloves, incontinence, and wound care supplies.
  • Hospice has transportation available to assist with doctor's appointments when the need arises.
